| Description: | Serves communities and rural areas of Christina Lake, Grand Forks, Greenwood, Midway, Rock Creek, Bridesville, Westbridge and Beaverdell. Provides specialized Mental Health services for adults age 19 and over (including the elderly) who have a serious and persistent mental illness such as schizophrenia, bipolar disorder, depression, anxiety disorder or a dementia and / or delusional disorder that causes significant disruption in one's ability to function. Services are provided by a multi-disciplinary team of professionals and include urgent / emergent response, assessment, treatment, consultation, education and advocacy. Using a case management model (ACT), services may be short term (1 - 10 sessions) or continuing. Groups may be used in conjunction with, or in lieu of, individual counselling. Referrals may be made by any concerned person, family member, professional or agency. All referrals are screened for appropriateness and urgency. There is no cost for services.
