| Description: | Serves adults with severe and acute mental illness by providing counselling, rehabilitation, family support and liaison to community resources. A multidisciplinary team includes life skills outreach workers, social workers, nurses and psychiatrists, who are available for assessment, treatment, case management, outreach support, education and residential placement. Individual and group programs available. Offers specialized mental health services to the elderly and / or their caregivers including specialized assessments of physical, social and psychiatric factors as well as treatment, consultation and follow-up (at home or in facility) from a multidisciplinary team. Other services for the elderly include comprehensive case management, individual counselling, specialized groups, psychiatric consultations, caregiver support and educational opportunities. Office hours: 8:30am to 12:00 noon; 1pm to 4:30pm
